For the past 25 years dermatologic surgeons have been pioneers in the advancement of liposuction under local anesthesia, otherwise known as micro-cannular tumescent liposuction. Accepted as a much gentler, safer, and refined technique, microcannular liposuction has become a standard of care amongst minimally-invasive cosmetic surgeons.

Even with the micro cannulaes that are used in tumescent liposuction, trauma may still occur to skin structures such as blood vessels, nerves and skin support tissues. Recently, advanced technologies such as VASER® Liposelection and SmartLipo ® have become available that further minimize skin structure damage and still dissolve fat cells. The VASER® Liposelection uses ultrasonic energy and the SmartLipo ® utilizes laser energy to accomplish these tasks. Both energies are delivered through small sterile instruments, advancing the tumescent microcannular technique to the next generation of precision and ease for both the patient and surgeon. With only local anesthetic and mild sedation, the fat is melted then gently massaged and suctioned out with tiny cannulas. No more visions of the jabbing, jarring and bruising of patients associated with traditional liposuction. A Friday procedure will have you back to work and play by Monday.

What Happens During Vaser & Smart Lipo? There are essentially three steps to an office liposuction procedure. First, the area selected is filled with a special saline solution that helps numb the site and shrinks blood vessels. Second, the Vaser and SmartLipo energy sources are used to liquefy fat cells. Third, the fat cells that have been liquefied are than removed during a gentle suctioning procedure. After suctioning, a pressure garment is put on and worn daily to tighten the skin and avoid any bruising. The small incisions heal quickly and easily.

How Does The VASER & SmartLipo Systems Work? The VASER System utilizes solid metal probes that are vibrating at a rate of 36,000 times per second. This generates an energy source at the tip of the probe. When this energy comes in contact with the fat cell, it breaks up the cell membrane and thereby destroys the fat cell. Fat cells are weak tissues and are easily destroyed by this vibrating energy. In contrast, blood vessels, nerves and skin support structures are relatively resistant to harm from this vibratory energy. Smart Lipo uses YAG laser energy that is delivered through a very small glass fiber. The laser energy breaks up fat cells through a pulse of energy as well as heating of the tissues. Nerves and skin support tissues are left unharmed and blood vessels are sealed closed to minimize bruising. Furthermore, the laser helps to tighten skin by stimulating collagen production. The combination of the two procedures makes liposculpting a safe and effective office procedure with maximum results and minimum downtime.

How do I know if I need one or both types of procedures? During the consultation with Dr. Zelko your areas of concern will be evaluated as well as your expectations. The recommendation of one or a combination of procedures will depend on the number areas to be addressed as well as the specific site and size of each site. As a general rule of thumb, if the area is a large area, ie. an abdomen, or a “thicker” area, the Vaser is used routinely. It is recommended to add the SmartLipo to all Vaser procedures for the added benefits of skin tightening and decrease bruising. Small areas can usually be done by using the SmartLipo only.
